Public Damage Reporting

In an effort to improve our response to a severe weather event or other incident, the Town of Davie is providing the following forms as a method for residents to provide situational awareness information ONLY for the Town’s Emergency Management Division about areas of damage. This information DOES NOT replace your responsibility to notify your insurance company and filing an individual claim if applicable. The Town of Davie will not be making insurance claims for individual residents.

If your home, property, business, or vehicle was damaged as the result of a severe weather event or other incident, please report this information to the Town via these online forms. Your assistance will assist the Town in directing emergency response assets, as well as requesting funding for Public Assistance. This information will also be helpful in mitigating future events or incidents. 

Please use this form to report the status and any damage to your residence or business. When complete, please select the Submit button at the bottom of the form. The information will be used to support the recovery efforts and remain confidential.
